Here’s the good news: your body gives you plenty of clues that labor is shifting from early to active. A cervical check is just a number. How you’re moving, coping, and feeling often tells us so much more.
As a doula, I’ve seen this over and over again, and with the right knowledge, you can learn to tune into those cues too.
Here are six signs you might be in active labor, no cervical check required:
In early labor, contractions often feel like mild cramps, and you can usually talk, walk, or even rest through them. Once active labor begins, contractions tend to:
This is one of my favorite doula tells. During active labor, birthing people often turn inward.
In early labor, you might distract yourself from contractions or cope with breathing, walking, watching Netflix, or resting. Once active labor begins, the intensity shifts and each wave asks for your full focus, often leading you to discover new ways to cope.

This “labor zone” is real. It’s not just in your head. Your brain is shifting into a different state as labor hormones like oxytocin and endorphins increase. That’s your body doing what it was created to do.
Your body often gives clues about what stage of labor you’re in. During active labor, you might notice:
Birth isn’t just physical…it’s emotional, too. In active labor, you may notice:
You’ll notice you’re working harder to get through each wave. That’s a sign labor is deepening. Your uterus is doing hard work.
Your body often shows little signs that things are progressing:
This one is powerful. Many moms just know when things have shifted. Even if contractions don’t follow a textbook pattern, your intuition often tells you it’s time to head to your birth space or call your doula.
Your body already knows what to do, whether or not a number confirms it. You don’t need external validation to trust that labor is unfolding as it should.
As a doula, I often remind my clients to notice how they’re moving, breathing, and coping. Those signs give us far more insight than any cervical check ever could.
I’ve supported moms who said, “It’s time to go,” and while the first check said 3 cm, they were pushing within the hour. I’ve also seen moms who appeared calm, trusted their instincts, and were already 9 cm and in transition when we arrived at their birth space.
Every body and every birth is unique. Trust your intuition. It’s more powerful than you realize.
Cervical checks are completely optional, and they rarely tell the whole story. You can be 5 cm and still in early labor, or 2 cm and well on your way. Dilation is just one piece of the puzzle, and your body can progress quickly or slowly, regardless of the number.
You have the right to decline cervical checks at any point. Some birthing people choose to have a check without being told the number, which can help them stay focused and calm. Most hospitals do require a cervical check upon admission, but after that, you can limit or refuse them based on your comfort.
Other observations, like effacement (how thin your cervix is) or baby’s station (how low they are in the pelvis), often give a clearer picture of labor progress than dilation alone.
At the end of the day, what matters most is how you feel. Your body is designed for this. Trust it. Trust your intuition.
The signs of active labor don’t look the same for everyone, and sometimes labor shows up differently than expected. Paying attention to your coping, movement, and emotions often tells us more than a number ever could. Cervical checks can provide information, but they’re just one piece of the puzzle.
Trust your body. It knows exactly what it’s doing.
